Umpire Sanction Application All Umpires must be sanctioned for District and State Tournaments. To become sanctioned, an Umpire must complete the application form below and mail it to the State Commissioner along with $20. After the State Commissioner receives the application an exam will be mailed. It must be completed and mailed back. Then the State Commissioner will notify the Umpire as to if he/she passed. If passed a sanction packet will be mailed. Application is in Adobe PDF format. Sanctions are good for a two year period
